I would like to know what make and model this will fit. Please use a reference link.

If you really want to know what this fits, I will need better measurements (unless you want a best guess.) Use your digital caliper set to inches and measure the shaft diameter (photo 7), the distance between the flats in the yoke (inside distance, kinda like photos 3 and 6), the diameter of the "hole" in your photos 4&5 above, and if you can get it, the length from the end of the shaft to the center-line of the two bores in the yoke.
