It's a torque multiplier wrench. You use a standard ratchet wrench in the square hole side. It looks like an old style Williams TM-1500. The newer version looks different.

Item History: Appears to be a gear reducer. When the Ratchet is turned the back side spins faster. Do not think it clicks or "ratchets".
